Gameplay[]

The game is very similar to the previous Heart's Medicine games, except there are many differences. Each level still has it's own challenges but instead of earning a diamond, you'll now earn ten diamonds and a puzzle piece for a certain character. For example on Shift one, if you complete the level and its challenge you'll earn a puzzle piece for Javier! Some challenges are easier to accomplish and are similar to some challenges in Heart's Medicine Hospital Heat and most of the challenges are new and some of the challenges are hard. Instead of earning three stars per level, each level can have up to five stars, Shift one to five and Guinea Pig level A1, Challenge level A2 and Time Trial level A3 are all tutorial levels which allow you to earn five stars easily.

There are now technically seven types of levels in this game:

Story levels are the main campaign levels about Allison's internship from the start to her graduation of becoming a doctor.

Challenge levels are special levels with an objective the player has to do, if they fail to complete the challenge, the level will still continue and the player can either retry the level to do the challenge or wait to do it another time. Challenge levels do not contain any story content.

Time trial levels are the same as Challenge levels, only the player can use the Next Patient button as many times as they wish before the timer runs out, these levels can easily earn all five stars if you spam the next patient button and serve patients quickly! Time Trial levels do not contain any story content.

Guinea pig levels are special levels that only require you to find Oliver the guinea pig and his friends Punky, Olive and Elly. Elly is only available in Elite mode. Guinea pig levels do not contain any story content.

Minigame levels are special levels which only have minigames and minigame levels do not contain any story content.

Seasonal Event levels are very special levels that usually last ten days during a seasonal time. Seasonal Event levels are used to unlock new story related to the season and contain 100 levels. Those 100 levels don't contain story content.

Daily Play levels are levels that are used to earn puzzle tokens to unlock the new story that continues after shift 60 in Chapter 7 and also chapter 8. Playing Daily Play Levels does not show you any story content but earning puzzle tokens lets you buy puzzle pieces for chapter 7 and beyond and also lets you unlock gifts on the gift road.

During later levels there are some treatments with multiple-choice selection, the player will need to select the correct one based on their request then after that the next request will appear. In room 2 (Chapter 2 or Diagnostics) and 3 (Chapter 3 or The Morgue) , some treatments produce medical waste chutes which can gain lot of score when emptied. In Room 4, the chute is back from the last two chapters, except this time it gives you more items for the vial, organ box and the skin sample items.
